Approval by the Board of Education of Amendment No. 1, Professional Services Agreement 2020-2021 between the District and Servio Consulting, LLC, Frankfort, IL, for the latter to reprogram COVID Daily Assessment survey to add conditional logic between questions to allow subsequent questions to be dependent on responses to previous question(s) across web, text and voice input methods, for the period of September 01, 2020 through June 30, 2021, in the additional amount of $2,900.00, increasing the Agreement not to exceed amount from $35,000.00 to $37,900.00. All other terms and conditions of the Agreement remain in full force and effect.
