Plan to use grant funds for 3 positions, Executive Director of Systems & Services Improvement, Director of Program Improvement and Assistant Project Manager for Systems & Services”, as delineated in Exhibit A of Amendment, incorporated herein by reference as though fully set forth, for the revised term of December 9, 2024, through March 30, 2026. All other terms and conditions of the Grant Agreement remain in full force and effect.
